Role Summary

  • To provide support to service users identified within their person-centred support plan
  • To undertake domestic and cleaning duties in the home whilst supporting service users independence, as identified within their person-centred support plan
  • To support service users with all activities of daily living and personal care including personal hygiene needs, nutrition, mobility and any other activity identified
  • To co-ordinate and facilitate group work
  • To provide and empower service users with budgetary difficulties and ensure they access the relevant services as required i.e. Credit Union
  • To undertake and support service users with shopping and cooking
  • To be proactive in supporting service users to engage with community activities, diversionary activities and therapeutic interventions to improve their health and well-being and reduce social isolation
  • To maintain contemporaneous computerised records and ensure care plans are accurate in accordance with the relevant policies and guidelines
  • The role requires flexible working. The hours will be generally 7am–10pm, seven days a week
  • To understand any other duties that are deemed necessary by the Team Leader or Service Manager
  • To have awareness and understanding relating to assistance technology and a willing to assist with its implementation
     

  • To provide support and relevant information in respect of the people we support to relevant partners following the relevant guidance and procedures
  • To be fully accessible during working hours to respond to the people we support needs
  • To conduct themselves in an appropriate manner when dealing with the people we support, Nurtrio staff and members, partner agencies and the public

  • To be aware of own training needs and bring these to the attention of the supervisor
  • To attend/complete all mandatory training including face-to-face, e-learning and distance learning
  • To develop a thorough knowledge of Nurtrio services available to the people we support and promote positive working relationships with them
  • To be aware of and adhere to Nurtrio/NAViGO policies and procedures
  • To participate in appropriate training courses and updates in accordance with mandatory requirements and/or individual personal development plans (PDRs)

Health and Safety

Under the Health & Safety at Work Act 1974 it is the responsibility of individual employees at every level to take care of their own health and safety at work and that of others who may be affected by their acts at work. Responsibility for Data Quality

All staff that record information have a responsibility to ensure that the data is relevant, accurate, complete, and captured in a timely manner so that it is fit for purpose. Staff must follow the agreed Nurtrio policies and procedures. A data subject (e.g. an employee or a service user) has a right to expect this under the Data Protection Act’s fourth principle: personal data shall be accurate and where necessary kept up to date

Respect Training

Nurtrio provides every member of staff with training appropriate to their work area in dealing, on occasion, with violent or aggressive behaviour. This training is designed to teach you de-escalation skills as a first intervention tool and physical techniques training to assist you in maintaining a safe environment if a situation arises. It is the responsibility of the post-holder to ensure and maintain training compliance.

Infection Control

All staff members have a responsibility to themselves, service users, visitors, and other staff to maintain high standards of infection prevention and control and follow the agreed Nurtrio policies and procedures. It is the responsibility of the post-holder to ensure and maintain ongoing training compliance.

Safeguarding

All staff members have a responsibility to themselves, service users and other staff in ensuring the effective safeguarding of children and adults. Staff must follow the agreed Nurtrio policies and procedures in both these domains. It is the responsibility of the post-holder to ensure and maintain training compliance.

Staff should ensure that their actions support the aim of the organisation in ensuring that all children up the age of 18 years are protected from significant harm, abuse, and neglect, in accordance with the multi-agency child protection procedures set out by the NEL Safeguarding Children Partnership and Nurtrio safeguarding policies.

  • The role involves working with clients who may have underlying problems with mental health, substance misuse and long-term conditions
  • The post holder will need to travel as and when required

  • The post holder may need to work longer hours when clients are in crisis

Working Conditions

  • The post holder will be working in a variety of settings, including people’s homes, other locations in the community, some of which will be in areas experiencing deprivation
  • The post holder will also work in a busy office environment, where there will be challenges to concentration and many diversions.

Confidentiality

  • In the course of your duties, you may have access to confidential material about service users, members of staff or other health service business. On no account must information relating to identifiable patients be divulged to anyone other than authorised persons, for example, medical, nursing, or other professional staff as appropriate. If you are in any doubt whatsoever as to the authority of a person or body asking for information of this nature, you must seek advice from the board.
  • Similarly, no information of a personal confidential nature concerning individual members of staff should be divulged to anyone without the proper authority having first been given. Failure to observe these rules will be regarded by your employers as serious gross misconduct, which could result in disciplinary action being taken against you. Individuals within Nurtrio Ltd are responsible for any records they create, use or handle.  This responsibility is established at, and defined, by law.
  • All employees working within Nurtrio who record, handle, store or otherwise come across information, have a personal common-law duty of confidence. The Data Protection Act 1988 now places statutory restrictions on the use of personal information, including health information. All staff need to acknowledge the importance of health records and their personal responsibilities.
  • Its security is of prime importance and serious consequences can result should a record go missing.  Any disclosure of such information without permission is a disciplinary offence and may result in dismissal.

Person Specification

  • A basic knowledge and understanding of health and social care 
  • GCSE or Level 2 Functional Skills in Maths, English, and IT or working towards these
  • Level 2 qualification in Health & Social Care, or willing to work towards a Level 2 qualification in Health & Social Care
